APPLICATION CHECKLIST



Deadlines: December 15th — Fellowship Consideration & Febraury 1st — Final Deadline

It is the applicant's responsibility to make sure that ALL materials are received by the application deadlines. Please contact the Student Affairs Office at (805) 893-2137 or via email at gradapp@education.ucsb.edu approximately 2-3 weeks before the deadlines to verify that all materials have been received. If you are re-applying, please notify the Student Affairs Office in writing using the address listed below or via email at gradapp@education.ucsb.edu.

Department Application Form.

Two (2) copies of your Statement of Purpose, including a description of personal achievements in and contributions to the field and a résumé or vita. Please do not staple the copies together.

Three (3) letters of recommendation. It is suggested that applicants submit more than three letters so that at least three letters are received by the deadline. If applicants require additional letter-of-recommendation cover sheets, photocopy them from the Graduate Study Application or download a copy from www.graddiv.ucsb.edu/admissions. Original letters are required. Facsimiles are not acceptable.

Official GRE scores (General test only) submitted directly from ETS to the UCSB Graduate Division. Use the following code so that OFFICIAL scores reach UCSB: UCSB institution code 4835.

Writing Sample. Applicants must submit a writing sample based on their professional experience.

Statement from Employer. Applicants must submit the Employer Agreement Form verifying a commitment from their current employer to provide periodic leave for intensive program activities.

Two (2) OFFICIAL transcripts from each institution attended since high school or secondary school. Applicants must make arrangements for these to be sent directly from their prior educational institutions.

$60.00 Check or Money Order payable to UC Regents.
