Psychology Assessment Center

How to Get Tested: Children (Ages 5-17)

 

We use a mail-in system for initiating testing services. To begin the process, open and print the following document:

It is important that you review our policies before you mail the form to us.

 

What happens next?

Once we receive your information, your information will be reviewed by Dr. Lande in order to ensure that we will be able to appropriately meet the needs of your child. If it appears as though you could be served by the PAC, you will be placed on our waiting list for the next available clinician.

Security Deposit:

You must send in an initial $100 non-refundable security deposit to hold your place on the waiting list. This payment will be credited to your account and count toward your final fee. Please make all checks out to "UC Regents." Referral forms sent without payment will be returned. Note that this security deposit is NON-REFUNDABLE unless we feel that your case is not appropriate for the PAC.

Waiting List :

Please note that the wait list for the PAC is generally between one and four months. If you have time pressures, please feel free to email us at pacucsb@gmail.com to find out how long the wait might be. The security deposit is not refundable in the event of an extended wait.
