Program & Credential Requirements Checklist 

For your convenience, we have created a TEP Program & Credential Requirements Checklist. This checklist will help you keep track of all of your state testing and other requirements that cannot be found on the eApp. This checklist is for your reference only and does not need to be submitted. 

Program and Credential Requirements Google Form

ALL supporting program and state credentialing documents should be submitted via our Program & Credential Requirements Google Form.

You may submit more than one copy of the Google Form, should you need to update the completion status of your requirements. Please view our Application Handbook for more information.

Due January 13, 2023 (with the application)

The following items are due on January 13, 2023.  Please submit using our Google Form when complete. Please view our Application Handbook for more information on each of these requirements.

• Basic Skills Verification
• Subject Matter Verification
• Financial Aid Statement (optional)
• Bilingual Authorization in Spanish Statement (optional)

Due July 1, 2023 (if admitted)

The following state requirements are due on July 1, 2023. Please submit using our Google Form when complete. Please view our Application Handbook for more information on each of these requirements.

• Certificate of Clearance 
  -- We recommend starting this process shortly after submitting your application.
• Tuberculosis Clearance
• CPR Certification
  -- There will be CPR certification classes held during Summer if you do not have your CPR Certification.
• Mandated Reporter Training
• U.S. Constitution Requirement
